Please download the 2025 Tax Questionnaire and the Engagement letter below, fill them out, and sign. If you use an auto for business, please also include a completed 2025 Auto Disclosure document.

The completed and signed forms can then be uploaded via our client portal, or include a physical copy with any paperwork dropped off with one of our offices. You also have the option to fill these out at the time of drop off - we will have printed copies available.
